Most of my photos are on Flickr under a Creative Commons attribution licence. That means anyone can use them for free, forever, including for commercial purposes as long as they credit me. The credit is important to me, and I do chase people who use them without credit. I assert my moral rights.
Sometimes a credit is awkward: packaging, an ad, a clean layout. The schedule below applies if you'd rather use one without attribution. The fee buys a release from the credit requirement and a short written licence you can keep on file. It's a bit under what you'd pay a stock library.

Images not under Creative Commons
Not everything I shoot is released under Creative Commons. On Flickr those photos are marked “All rights reserved,” and some aren't published there at all. There's no free route for these: any commercial use needs a licence, not just use without a credit.
The same schedule applies. I just charge it from the first use rather than only when you want to drop the attribution. If you're not sure whether an image is Creative Commons, ask me and I'll tell you.
